Mr. Allen smith is a 81 yrs old male who is long term SCI resident. He has the ability to respond to pressure related discomfort. Patient is alert and oriented. Slow to respond at times and has difficulty finding his words or expressing his feelings. Patient has foley catheter in place draining yellow, clear, urine. Patient is aware of his own limitations and understand to call help when needed. Bed Siderails upx4 – patient’s preference. Heels offload with pillow. Slept well through the night. Non-compliant of turning every 2 hours.
Pain in lower extremities. Braden scale is 11 which is high risk for pressure ulcer. Patient has weak limbs upper and lower extremities.
