For this course, it is important to have an understanding of the Social Determinants of Health (SDOH) as they relate to an individual’s health and functionality
and the importance of being able to support the following: assessment of local resources, social services, and programs. Choose one case study from the
following sources:

Access one of the Case Studies through the University of Utah and the training for Health Equity Network – Targeting the Social Determinants of Health in
