

Assignment 2. Evaluation Questions, Outcome, & Process Indicator Brief
(Objective 3) – 15 points
After learning about a local program, students will take on the role of that program’s evaluator to develop evaluation questions and outcome and process indicators to answer those questions.
This assignment builds on the Logic Model you created for the FRESH or YMCA DPP program, as assigned.
Focus your evaluation efforts on the outlined in the USDA grant proposal for FRESH program; and website materials on YMCA DPP.
Each evaluator (student) will submit a 3 to 5 page document. Your document should include five evaluation questions, with a paragraph for each question describing:
• why this question is important (rationale)
• who cares about this question (stakeholders)
• what program component this question addresses (program description)
• how and when it will be measured, with at least one (labeled!) process and outcome indicator
Your question and indicator together should form a SMART question.
