“Ms.S” is a 30-year-old single Hispanic woman. She was seen by her PCP in a community
health center for a routine physical. The exam revealed no major health issues other than a
paternal history of hypertension and a maternal history of diabetes. Her lipid panel, blood
pressure, and Papanicolaou smear were all normal, and her thyroid test results were pending.
Ms. S told her PCP that she has been sleeping more than usual and has been having a hard
time getting up for work in the mornings. She also felt as though she had gained some weight
lately despite not eating more than usual. Ms. S reported that she does not smoke or use
recreational drugs, and she exercises only on occasion. While she said that she knows she
should exercise regularly, she “just can’t seem to find the motivation”. She denied any suicidal
ideation and reported that although she had been late to work a few times, she was generally
able to function well in her role as manager in a large department store, despite some friction
with her supervisor. In talking about this, Ms. S’s becomes tearful and admits that she feels
overwhelmed by everything lately.
You are a social worker in the community health center and receive a call from the PCP who
requests that you meet with Ms. S. In 2 weeks, you will report back to the multidisciplinary team
(including the PCP) about your assessment of Ms. S.
