Identifying Data
The customer was a 31-year-old American citizen who visited our offices wearing black gym clothes with unkempt hair that seemed to be a casual ponytail. From the appearance, she looked scared, worried, anxious, depressed, and exhausted. While speaking to her, she often avoided eye contact and kept on fiddling. She would look down regularly, crying emotionally to lack words of expressing her feelings.
Presenting the Problem
The customer was advised to visit our clinic by her friend, Karen. This is after she was not able to cope with the separation from her husband, who practically abused her several times. The client had emotional issues since she could not eat nor sleep, thus losing weight due to this Post-trauma (Qaseem et al., 2016). The main problem was that her mother was against her move to come out of her husband’s house, but she is committed to providing the best for her children.
Identifying the client’s vulnerabilities by considering why the client was more likely to experience this problem(s) than another person (2 Points)
Irene’s marriage has never been a smooth way as thought. She experienced the challenge often since the beginning is elaborated by fierce attempts by his husband, where she demonstrates and says that he would push her occasionally during heated conversations. The situation had worsened before she decided to go back to her mother’s home, with her husband now turning verbal and physically abusive. The man turned violent after starting alcohol drinking. From experience, women married to alcoholic men are to higher probability vulnerable to household quarrels. For our client, she explains of a loving and caring husband who turns into a different person and foregoes the value of the marriage and graduates into an excessive alcoholic. However, the alcoholism effect is not felt by the children but the wife. The husband, on one occasion, slapped the wife before the children and expressed his discontent with such a worthless wife. Now, the client is experiencing insomnia and post-trauma stress disorder after this exploitive relationship and the fact that her mother is against the decision to leave the family. Indeed, she is stressed to the extent of lacking sleep and would wake up at midnight due to lack of sleep and fall off during the daytime when she feels to sleep. On several occasions, her heart pounds, making her experience chest pains and breath shortage. The core problem and vulnerability of the client is the dilemma on the welfare of her children against the abusive relationship.
Identifying the client’s triggers by considering the stimulus or source of the presenting problem(s) 2 points
The client is pushed by the sudden change of her husband to come and present the issue at least to see if she could receive some help. The alcoholic and cruel behavior of her husband is an annoying and stressful factor which she takes as the core problem to this marriage. During the interview and conversation process, she is constantly crying as she is over emotional (DeKeseredy, Dragiewicz, & Schwartz, 2017). She is indeed stressed by the dilemma and concern of her children, as she would never imagine at a point having the children denied parental love. She is showing signs of loss of weight, body exhaustion, and PTSD, as this is a great menace to her love life.
The pessimism of her mother due to her decision is another worsening factor. The mother is in support of the marriage and would not imagine her daughter divorced. This situation makes the client worse since she cannot imagine the conditions and mistreatment of her marriage.
What is the pt? Core Beliefs (4 points?)
During my interview with the customer, I could understand the feelings and points of discontent according to her feelings. The client is ever regretting stating phrases like ‘I wish I knew this is how the marriage would be.’ She continuously applauds her mother, but regrets she could not please her anymore considered the pain she is undergoing. She talks of a pool of supportive friends and their words of encouragement that they are offering to her and encouraging her to take heart and consider reviving her marriage life. When she pronounces marriage life, she weeps uncontrollably and expresses much regret. She is bitter of her decisions, bitter of how the friends would take her, sorrowful on well being of her children amidst the family separation. As we negotiated, I could get the best techniques to use as the diagnosis of the client. I could suggest the most probable causes of the problems and increasing body weight loss.
What are the most central thoughts, feelings, memories, sensations, and situations that the client is fused with or avoiding? (2 points)
Irene is overwhelmed since she has great treasure in her children and wish to offer them the best in life. She as well feels that she is not living according to the wish of her mother, who wants her to live a happily married life. The client could not imagine failing to live according to her mother’s teachings during her upbringing. She is also devastated by the fact that she even has girls looking up to her as a good image in their lives. These conditions are putting her in a significant dilemma such that she no longer wants herself to be associated with life. In fact, at one point she stated that it would be better to be dead than alive through such sufferings. She could not deny missing the life moments they shared when their marriage was new. The moments of thinking are a hurting condition to a greater extreme. She neither wants to be associated with the above things nor factors that bring her back to the times.
Differential Diagnosis (0.5 points)
From the tests I conducted to establish the root of the problem my client is experiencing, I realized that she was suffering from Major Depressive Disorder and Insomnia (Kupfer, Frank& Phillips 2016). The two conditions continued to worsen to a point whereby the lady had to take coffee to reduce stress and sleep. She exhibited signs of body exhaustion and needed to be given some dose to boost her appetite to avert the possible negative impact of her condition.
Diagnosis (Please elaborate)
The diagnosis stage was significantly influenced by the expressions of the client during our communication process. The lady exhibited much pain when she remembered the experience she underwent. Emotions and sorrow took a better part of her upon mentioning her marriage during the conversation. She could not withstand the communication when it reached that point.
Exploring coping strategies by considering how the client deals with the effects of the presenting problem(s) 2 points,
the client explained various techniques she employment to save her marriage before she felt it was beyond her capacity. She, at many times, did not answer to allegations from her drunk husbands until the following day when she pleaded with him to stop or reduce the habit of drinking alcohol. This failed to work despite her combined effort for a long time. She also convinced her husband’s friends to talk with him to stop being violent. She even apologized for what convicted against her despite not having done it, but all this failed to work.
Finally, describe a comprehensive treatment plan which considers the particular processes, interventions, and measures you might want to use with this specific client
The client has exhibited signs of Major Depressive Disorder and needs to establish a method of treating the disease. This is since she has shown signs of loss of appetite, low energy levels, lack of sleep, low esteem, and decreased levels of concentration. She has a well-shown interest in committing suicide. I would use the Rogerian or person-centered treatment plan as the best method to cope with this client problem. The model of treatment states that human being has a drive for growth and have the best wish or the community if put in a caring and accepting environment. From the discussion, I found that Irene is a caring and loving woman, and if not for the challenges she has undergone, she could shine and produce the best of her in society.
